Patients sometimes need paperwork: a school form, a brief work note, or a letter summarizing treatment. Those documents are a clinical task, not a product you can add to checkout.
When it is clinically appropriate
$25 extra when completed during a scheduled visit, in addition to the visit fee. $75 when completed outside a visit. The same prices apply with either provider. A request can be declined if the record does not support what is being asked, if more evaluation is needed, or if the document would be misleading.
What this is not
It is not a guarantee of a 504 plan, IEP, ESA letter, FMLA result, or court opinion. Schools, employers, housing providers, and agencies make their own decisions. A psychiatric clinician can only speak to what the evaluation actually shows.
Adults transferring care should bring prior records rather than expecting a letter on the first day.
